Key Points 90 yr old lady with circumferential extensive recurring leg ulcer of mixed aetiology. Previous wound swab indicated pseudomonas and heavy haemolytic Strep G, later developed cellulitis. Initially dressings being changed twice daily and strikethrough apparent within few hours of dressing change.

Key Points Mixed aetiology traumatic leg ulcer 17cm x 20cm, large amounts of slough, clinical signs of infection including increased exudate, pain and erythema. Patient refused antibiotics. Suprasorb X+PHMB applied with a foam. After 3 days patient had no pain, visible signs of granulation and decreased redness.

Key Points The management of a lady with long-standing, chronic arterial leg ulcer heavily infected with Pseudomonas aeruginosa - a frequently isolated pathogen within chronic wounds. The wound was 100% green/yellow thick slough and the lady had a VAS score at worst of 8/10. There were high levels of odour and exudate. Previous treatment included iodine and silver sulphadiazine cream with reduced compression.
